In
order to obtain a passport the following is needed

Renewal of
passports: Every two (2) years after the date of
issuing up to six (6) years.

Change of
passport book: Six (6) years after the
expiration date. Four photographs 2"x2" are
required and the expired passport book.

First time
issuance or issuance for loss: "Cedula"-ID Card,
legalized birth certificate and four photographs
2"x2" are required.

Change of civil
status: Marriage certificate or divorce
certificate, depending on the case. |
Passport insurance and renewals should be
accompanied by Form SP/822 completely filled
out (typed or in black ink or blue ink) that costs
US$0.50 + 12% Law 5113 that is US$1.00.
Issuance of a passport book: 6 years US$91.50/4
years, US$71.50/ 2 years
US$51.50.
Renewal
of passport book: 6 years US$71.50; 4 years
US$51.50; 2 years US$31.50.
In the
event that a passport is lost or stolen or if a new
passport is needed a consul can issue a replacement
passport.
If you
believe your passport has been stolen, first report
the theft to the local police and get a police
report.
If passport is stolen and you do not have the police report you need a solemn declaration signed by the Consul and there is an additional fee of US$100.00 for this document. You will need proof of Dominican citizenship (a previous passport, birth certificate, naturalization certificate).

Honorary Consulates are not authorized to issue
passport books, and can only renew these following
receiving authorization from the Consular Department
of the Ministry of Foreign Relations.
Passport Loss Certificate
A
certificate of the declaration of the bearer will be
issued.
