IN ORDER TO OBTAIN A DOMINICAN PASSPORT THE FOLLOWING IS NEEDED

Renewal of passports: Every two (2) years after the date of issuance and up to six (6) years.

Change of passport book: Six (6) years after the issuance date. Four photographs 2"x2" are required and the expired passport book.

First time issuance or issuance for loss: ID Card (Cedula), legalized birth certificate and four photographs 2"x2" are required.

Change of civil status: Marriage certificate or divorce certificate, depending on the case. |
Passport issuance and renewals most be accompanied by Form SP/822 completely filled in (typed or in black ink or blue ink).
In the event that a passport is lost or stolen or if a new passport is needed a Consul can issue a replacement passport.
If your passport has been stolen, report the theft immediately to the local police and get a police report.
If passport is stolen and you do not have the police report you need a solemn declaration signed by our Consular Section. You will need proof of Dominican citizenship (a previous passport, birth certificate, naturalization certificate).
Honorary Consulates are not authorized to issue passport books, and can only renew the following receiving authorization from the Consular Department of the Ministry of Foreign Relations of the Dominican Republic.
Passport Loss Certificate
A certificate of the declaration of the bearer will be issued.
